My favorite invite wordings tend to be a little cutesy (it’s just my style I guess:), so in your situation I’d probably go with something like:

On September 26th, 2009
The beaches of Jamaica
became even more beautiful
With our promise of never-ending love and faithfulness
Please join us as we now celebrate
the blessing of being husband and wife!

You are invited to attend the reception
of the newly married
Mr & Mrs Kathy Jones
etc, etc.

The wording of the first answerer is great, but you need to add at the bottom “No Gifts, Please” – since these people weren’t at the ceremony.
Wish you luck!
I would not include any mention of the birthday at all, though. You can announce something at the dinner.

My sister got married last August, but it was on a weekday and a just family ceremony. They had reception with friends and all the extended family the following Saturday.

The invite was something like this:

Jane Smith
and
John Lock
Will be married on Thursday August 14, 2008 in Toronto, Ontario

They request your presences in celebration at reception on Saturday, August 16, 2008 at the Brant Park Inn Hotel, in Brantford Ontario.

…. the address, etc

You can make it as formal or casual as you want, but since you will have to mail the invites BEFORE you will actually be married, you kind of announce that you are getting married (and the date) but then INVITE the guest to the reception.

It depends on who is footing the bill/hosting.

let’s say it’s you and your husband:

yourfirstname yourlastnamebeforemarriage
and
hisfirstname hislastname

request the pleasure of your company at a reception celebrating their recent marriage

on thedate
at thetime
thelocation

add a response card.

Let;s say you want your mom to be the official host, as it is her house and she may be paying.

Ms./Mrs. herfirstname herlastname

requests the pleasure of your company/honor of your presence at a reception celebrating the recent marriage of her daughter

yourfirstname yourlastnamebeforemarriage

hisfirstname hislastname

(and while this is option, and esp. unnecessary because he is indeed 40, but can be nice for his parents, esp. if their friends are invited)

Mr. and Mrs. hisdad’sfirstname hisdad’slastname

thelocation

People can ask about when you get married when they see you or respond.

A general note: registry information is NEVER included in a polite invitation. People can ask for it if they want it.

well ive never actually been married but i would think you would say something like your ivited to my wedding reception. then say like where its at, when it is, rsvp, why, who, then say hope you can come.

The pleasure of the company of
[write each name, no &family or &guest stuff]
is requested at a dinner dance
to celebrate the marriage of
[bride's name], daughter of
[names], to
[groom's name], son of
[names]
at Six O’Clock … etc etc etc

It isn’t necessary to mention when exactly the marriage being celebrated occurred. I’d omit mentioning the birthday and save that part as a surprise. You don’t want your guests to believe that you are fishing for extra gifts.

Here is the wording my friends used to host a reception here in town after their destination wedding in Puerto Rico:

(Bride) and (Groom) were married in an intimate beach ceremony in (Jamaica) on (date). You are invited to attend a reception in their honor on (date) at (location). We will also be celebrating the 40th birthday of (groom). Please RSVP by (date)
